An alternative solution to dealing with these problems is to use videoconferencing (think Skype) instead of voice. It is true that a phone system is only as good as the quality of the voice connection you have to it. That means if you are relying on Skype for voice, you need high quality IP connections to your users, otherwise your voice quality is reduced. But if you are using videoconferencing as the primary means of communication, then you can have lower quality IP connections, and of course some of your clients will be on computers with very low bandwidth connections to the internet. VoIP trunks are usually many times slower than VoIP satellites, however they can be used to carry SIP traffic. A SIP trunk will not be able to connect to a PSTN directly. You have to provide a PSTN gateway for it to connect to.
Is an IP phone set up and working with the router? If this is the case, and the phone is connected to the router via Ethernet, the router should be able to handle voice and video calls. If it cannot, then the phone needs to be configured to use a different port. If you are using wireless phones, you need to be sure that the router can handle wireless.
